North Texas health departments are using an extensive questionnaire to try and track down the source of a food-borne illness.
The number of cyclospora cases continues to rise.
Patients who test positive for the intestinal illness are being given a 16-page questionnaire developed by the Centers for Disease Control. They're being asked about specific restaurants where they've eaten, and where they have purchased certain vegetables and fruits.
